CORESS confidential reporting app.

CORESS is an independent charity, which aims to promote safety in surgical practice in the NHS and the private sector.

This anonymised reporting app for surgeons and surgical teams enables you to efficiently supply CORESS with confidential incident reports. These reports are analysed by the Advisory Board, who make comments, identify lessons learned, and publish the results so that others can learn from them.

The app allows easy reporting, in complete confidence, by recording, transcribing, editing, and submitting to CORESS straight from your phone.

You can submit your report completely anonymously, but we would prefer you to identify yourself so we can contact you if there are any queries about your report. Every submission is treated in the strictest confidence and when published, will never contain any clues as to its origin. We will only keep your personal details if you give us permission.

Record your report or type it if you prefer. There is no dictation time limit and recording can be paused and resumed later.

Make sure Low Power Mode is not selected on your device.

When recording an incident, do not include identifier names.

Say ‘comma’, ‘full stop’, ‘new paragraph’ to punctuate your report.

When recording is completed, you will be presented with a transcript that you can edit. You can also include causative factors using a simple drop-down list. A summary of the lessons learned can also be recorded, and time/date of incident captured.

Using this app will help CORESS educate and avoid blame. When you recognise a near miss or adverse event, react by taking action to stop it happening and then report the incident so that the lessons can be published.
